What If Your Brain Is Actually Two Brains in One?

We usually think of the brain as one organ. Different areas may control different things, but they are all supposed to be pieces of the same system. New research from Stanford Medicine is challenging that idea. One system forms the forebrain and midbrain, including areas involved in language, reasoning, consciousness, memory, and complex thought. The other forms the hindbrain, which controls some of the functions we rarely have to think about, such as breathing, heartbeat, sleep, swallowing, and movements of the face and throat. Researchers discovered that these two regions trace back to different types of early embryonic cells. Even more surprising, versions of this split can be found across hundreds of millions of years of evolution.

This discovery may sound like an interesting biology fact, but it could have important medical consequences. Scientists have struggled for years to grow certain hindbrain neurons in the lab. Once the Stanford researchers realized that these cells begin through a different developmental pathway, they were able to produce functional human hindbrain motor neurons from stem cells. That could give researchers a much better way to study diseases such as ALS and spinal muscular atrophy, which damage neurons involved in movement, swallowing, and breathing.

What I find most fascinating is how much we can still learn about something as familiar as the human brain. We have mapped its regions, scanned its activity, and studied it for centuries, yet a basic assumption about how it forms may have been wrong. The brain sitting inside your head may look like one organ, but its story appears to have begun as two very different systems that evolution eventually brought together. Sometimes the biggest scientific discoveries do not come from finding something completely new. They come from realizing that something we thought we understood was much more complicated all along.

A cancer biopsy slide can already tell a pathologist a lot. Under the microscope, the shape, size, and arrangement of tumor cells can reveal what type of cancer is present and how aggressive it may be. But what if the same slide could also hint at the cancer’s genetics? That is exactly what researchers are trying to do with artificial intelligence.

A recent study published in npj Digital Medicine introduced an AI system called HE2FISH, designed to analyze routine H&E-stained biopsy slides from patients with large B-cell lymphoma. Instead of only looking at what the tumor cells look like, the system searches for subtle visual patterns linked to genetic rearrangements involving MYC, BCL2, and BCL6. These genes matter because certain rearrangements can be associated with more aggressive forms of lymphoma. Doctors usually identify them using a specialized laboratory test called FISH, or fluorescence in situ hybridization. FISH is an important diagnostic tool, but it requires extra laboratory work, tissue, time, and resources.

HE2FISH takes a very different approach. It does not directly test the genes. Instead, the AI studies the digital image of the biopsy and tries to predict whether those genetic changes are likely to be present. What makes the study especially interesting is its scale. Researchers evaluated the model using 1,377 patients from five hospitals. When tested on patients from hospitals outside the original training data, the system still performed well, reaching an AUC above 0.81 for individual gene rearrangements.

That does not mean AI is replacing pathologists or genetic testing. Far from it. The technology would be more useful as an early screening tool, helping doctors identify which patients may need confirmatory molecular testing sooner. Still, the idea is powerful. A routine biopsy slide may contain far more information than we can recognize with the human eye alone. AI could help uncover some of those hidden clues. The future of pathology may not be just about looking at cells under a microscope. It may be about combining the experience of pathologists with AI to read deeper into the biology of cancer.

Pancreatic cancer has a reputation. Doctors say the name and something shifts in the room. It’s usually caught late, it moves fast, and for decades, nothing doctors threw at it made much of a dent. So when I heard a new drug nearly doubled survival in a recent trial, I had to stop and actually dig into what happened.

The trial is called RASolute 302, and the drug is daraxonrasib. According to results presented at the 2026 ASCO Annual Meeting and covered by the ASCO Post in June, patients taking it lived a median of about thirteen months, compared to just seven on standard chemotherapy. Read that again. Almost double. For a cancer notorious for stealing time, that’s not a small win, that’s borrowed years handed back.

Here’s the twist: the reason pancreatic cancer has been so brutal to treat comes down to a single gene, RAS. It drives nearly every case, but it’s got this smooth, slippery shape that made it basically impossible to grab onto. Scientists tried for decades, failed for decades, and eventually just gave up and slapped a label on it: “undruggable.” It became the villain everyone knew about and nobody could touch.

Daraxonrasib walks right up to that villain and doesn’t blink. Instead of chasing one exact version of the mutation like earlier drugs tried, it blocks RAS wherever it’s active, in whatever shape it’s hiding in. It even worked in some patients whose tumors had no detectable RAS mutation at all, which is almost unheard of for a targeted drug. Oh, and it’s a pill. Not an IV, not a chair, not an afternoon plugged into a machine. Just a pill.

Researchers are already testing whether it works even better as a first move, before chemo ever enters the picture. Nobody’s calling pancreatic cancer “curable,” that would be reckless. But for the first time in a long time, people in the field are saying “transformative” and actually meaning it. When a cancer this stubborn finally cracks, even a little, that word earns its place.

Here’s a wild thought: what if the cure for cancer was already inside you, just waiting to be pointed in the right direction? That’s not science fiction, it’s basically what CAR-T cell therapy does, and we’ve had it for almost a decade now. Doctors pull out a patient’s own T cells, the immune system’s frontline fighters, and re-engineer them in a lab to recognize cancer cells like a wanted poster. Then those modified cells go back into the body and start hunting, multiplying as they go, taking cancer down one cell at a time.

What still blows my mind is how personal this treatment actually is. This isn’t some pill mass-produced in a factory. It’s built from a patient’s own blood, rebuilt into something new, and handed back to them like a weapon they didn’t know they had. Doctors sometimes call it a “living drug,” and that’s not just a cool phrase, it’s literally true. The cells keep working and multiplying long after the treatment day is over, quietly finishing the job.

The first version got the green light in 2017 for certain blood cancers, and according to a review published in the American Journal of Managed Care this past July, it hasn’t stopped expanding since, now covering multiple myeloma too. Here’s the part that actually gets me: some of the very first patients treated are still cancer-free, years later. For people who had already run out of options, who’d been through chemo, radiation, every last-resort treatment available, this wasn’t just another drug. It was extra birthdays. Extra holidays. A future that wasn’t supposed to exist.

But let’s be honest, it’s not magic, and it hasn’t cracked cancer wide open. CAR-T thrives against blood cancers because those cells are out in the open, floating through blood and bone marrow, easy to find. Solid tumors, like the ones in lungs or the pancreas, play a completely different game. They’re physically harder to reach, and they’re sneaky, they build a kind of immune blackout zone that keeps engineered T cells from getting in. Scientists are racing to crack that problem, while also trying to make the treatment cheaper and faster, since right now, every single batch is custom-built for one person, and that takes weeks and a serious amount of money.

There’s a riskier side too, one that doesn’t always make the headlines. Because CAR-T cells hit the immune system so hard, some patients experience something called cytokine release syndrome, basically the immune system going into overdrive all at once. It’s usually manageable, but it’s exactly why this isn’t a walk-in-walk-out treatment. Patients need to be watched closely while their own bodies fight this internal war.

Ten years in, CAR-T hasn’t solved cancer. But it proved something no one could prove before: your own immune system, with the right push, can become a real weapon. And once science proves an idea like that actually works, not in theory, but in real people, in real remission scans, it’s nearly impossible to walk away from it.

Two patients can receive the same diagnosis, take the same medication, and follow the same treatment plan—yet experience completely different results. One may recover quickly, while the other improves only slightly or develops unexpected side effects. This variation is one of the reasons medicine is rarely as simple as matching a disease with a treatment.

Genetics can influence how a person responds to medication. Small differences in genes may affect how quickly the body absorbs, breaks down, or removes a drug. A standard dose may work well for one patient but be too weak or cause stronger side effects in another. This is one reason researchers are interested in pharmacogenomics, the study of how genes influence responses to medications.

Age also matters. Children are not simply smaller adults, and older patients may process medications differently because of changes in kidney function, liver function, metabolism, and body composition. Older adults are also more likely to take several medications at once, increasing the possibility of interactions.

A patient’s environment and lifestyle can shape treatment outcomes as well. Nutrition, sleep, exercise, stress, smoking, alcohol use, and exposure to pollution can all affect the body’s ability to recover. A treatment plan may be scientifically appropriate, but its success can depend partly on what happens outside the clinic.

Psychology can also influence how symptoms are experienced and how closely someone follows a treatment plan. Fear of side effects may cause a patient to avoid taking medication. Depression may make it difficult to attend appointments or maintain daily routines. Stress can affect sleep, pain, inflammation, and other physical processes. These influences do not mean that an illness is “only psychological.” They show how closely the mind and body are connected.

Communication is another important factor. Patients need to understand why a treatment was prescribed, how to use it, and what side effects deserve attention. Medical instructions can become confusing when they involve unfamiliar terminology, rushed appointments, or language barriers. Even an effective treatment may fail if a patient does not receive a clear explanation.

Access to care can create even greater differences. One patient may be able to see a specialist, obtain medication immediately, attend physical therapy, and return for regular follow-up visits. Another may face high costs, limited transportation, long travel distances, or an inflexible work schedule. The diagnosis and prescribed treatment may be identical, but the conditions surrounding recovery are not.

Other medical conditions can also change how a treatment works. A therapy selected for one disease may affect another condition or interact with a medication the patient already takes. Clinicians must consider the whole medical history rather than treating each diagnosis as an isolated problem.

This complexity is why personalized medicine has become such an important area of research. Medical data, genetic information, and artificial intelligence may help clinicians predict which treatments are most likely to benefit particular patients. Instead of relying only on the average response observed in a large group, medicine may become better at accounting for individual differences.

Personalization does not mean that an algorithm should make treatment decisions alone. Data can help identify patterns, but clinicians must interpret those patterns in the context of a real person. A patient’s preferences, responsibilities, concerns, and goals may be just as important as laboratory results when choosing among reasonable options.

Medical treatments are often evaluated by asking whether they work on average. That evidence is essential, but an average cannot describe every patient. Some people respond exceptionally well, some experience little benefit, and others cannot tolerate the treatment at all.

The challenge is not simply to discover treatments that work. It is to understand which treatment is most likely to work for a particular patient, under that patient’s actual circumstances. The same diagnosis may appear in two medical records, but the people behind those records are never exactly the same.

Medical data can appear objective. Laboratory results are numbers, medical images record what a scanner detects, and health records contain diagnoses, medications, procedures, and outcomes. When thousands of these records are combined, researchers can identify patterns that would be impossible to see in a single patient.

Artificial intelligence can analyze this information at an extraordinary scale. AI may help recognize disease in an image, estimate a patient’s risk of complications, interpret speech, or identify people who need urgent attention. But an AI system can learn only from the data it receives. If those data do not represent the patients who will use the system, some people may be left behind.

A medical dataset is not a complete picture of every patient. It represents people who entered certain healthcare systems, received particular tests, and had their information documented. Patients who lacked insurance, transportation, nearby hospitals, or access to specialists may be missing. Even within a dataset, some ages, languages, accents, skin tones, disabilities, and medical conditions may be represented far less often than others.

This matters because AI learns from examples. If a model repeatedly encounters one type of patient, it may become very good at recognizing patterns in that group. When it encounters someone who was rarely represented during training, its predictions may be less reliable.

Consider an AI system designed to identify a skin condition from photographs. If most training images show the condition on lighter skin, the model may struggle to recognize how it appears on darker skin. A voice-based medical system trained mainly on clear recordings from native English speakers may perform differently when a patient has another accent, speaks a different language, has a speech disability, or is struggling to talk because of pain.

The system could still report impressive overall accuracy. Suppose a model is tested on 1,000 patients, but 900 belong to one well-represented group. Excellent performance for those 900 patients could hide poor performance for the remaining 100. This is why researchers must ask not only, “How accurate is the model?” but also, “Accurate for whom?”

Medical data can also reflect inequalities already present in healthcare. One patient may have frequent medical visits because they have insurance, transportation, and flexible work hours. Another person with the same condition may appear less often in the records because receiving care is more difficult. If an algorithm interprets the number of previous visits as a simple measure of medical need, it may misunderstand both patients.

Missing information can create similar problems. A blank field might mean that a test was unnecessary, unavailable, performed elsewhere, or simply not documented. Unless researchers understand how the data were produced, an algorithm may learn patterns that do not mean what they appear to mean.

These limitations are not reasons to reject medical AI. They are reasons to develop it more carefully.

Researchers can build more representative datasets by working with different hospitals and communities. They can evaluate performance across patient groups instead of relying only on one overall accuracy score. Systems intended for emergency departments should be tested in noisy, unpredictable clinical environments—not only with clean research data.

AI should also communicate uncertainty. If a system encounters a patient who differs greatly from the examples used during training, it should indicate that its prediction may be less reliable. Clinicians can then ask additional questions, order another test, or decide that the automated result does not fit the patient’s situation.

Human oversight remains essential. AI can process information quickly and recognize patterns across millions of records, but clinicians contribute context, judgment, and responsibility. Patients should also understand when AI is being used and what role it plays in their care.

Working with medical data has taught me that building a model is only part of the challenge. A high accuracy score may be exciting, but it does not tell us who was represented, who was missing, or how the system will respond to a patient unlike those it has seen before.

Medical AI has enormous potential to support earlier detection, improve communication, and help clinicians manage complex information. Realizing that potential requires examining the gaps hidden inside the data.

The most important question is not simply whether an algorithm works. It is whether it works for the patient standing in front of us.

When a patient develops unusual symptoms, we often imagine that testing will quickly reveal the cause. For people with rare diseases, however, finding an answer can take years.

The first challenge is recognition. Doctors regularly encounter common illnesses but may never see a particular rare disease during their careers. When symptoms such as fatigue, pain, weakness, or digestive problems appear, it makes sense to investigate more common causes first.

Rare diseases can also look different in different patients. The same condition may cause severe symptoms in one person and subtle symptoms in another. It may affect several parts of the body, sending a patient to multiple specialists who each see only one piece of the problem.

Testing does not always provide a clear answer. Standard blood tests and medical images may show that something is wrong without identifying the cause. Some conditions require specialized testing or genetic sequencing. Even genetic tests can produce uncertain results because scientists are still learning what many genetic differences mean.

Medical information may also be scattered across hospitals and specialists. A symptom documented years earlier may seem unrelated to a new problem, even though both are caused by the same condition. This prolonged search is often called a “diagnostic odyssey.”

Technology may help shorten that journey. Genetic sequencing can examine many genes at once, while artificial intelligence can search medical records for combinations of symptoms that might otherwise appear unrelated. Shared patient registries can also help clinicians compare rare cases across different hospitals.

These tools still require human judgment. An algorithm may suggest a possible diagnosis, but a clinician must determine whether it fits the patient and which tests should follow.

Diagnosing a rare disease requires curiosity and persistence. Common explanations should be considered first, but symptoms should not be dismissed simply because they do not fit a familiar pattern.

A diagnosis may not immediately provide a cure, but it can end years of uncertainty, guide treatment, connect families with specialists, and finally give a name to what the patient has been experiencing.

A placebo is often described as a treatment with no active medical ingredient. It might be a sugar pill, a saline injection, or a procedure designed to look like a real treatment. If it does not directly target a disease, why do some patients report feeling better after receiving one?

The answer begins with expectation. When a patient believes a treatment may help, the brain can change how it interprets signals coming from the body. This does not mean the symptoms were imaginary. Pain, for example, is influenced not only by tissue damage but also by attention, emotion, memory, and previous experiences.

The brain also learns through association. If taking medication has repeatedly been followed by relief, the act of receiving treatment may itself create an expectation of improvement. The appearance of a pill, a clinician’s explanation, and the healthcare environment can all influence that response.

However, a placebo does not cure the underlying cause of every condition. It cannot eliminate an infection, repair a broken bone, replace insulin, or shrink a tumor. Its effects are generally more noticeable for symptoms that involve how the brain processes sensations, such as pain and nausea.

Not every improvement in a placebo group is necessarily caused by the placebo effect. Symptoms may improve naturally, patients may change their behavior, and supportive attention from clinicians may make a difference. Researchers must separate these factors when evaluating a treatment.

Expectations can also produce the opposite response. The nocebo effect occurs when fear or negative expectations contribute to unpleasant symptoms. A patient who is extremely worried about a side effect may become more likely to notice or experience it. This is one reason the way clinicians communicate risks matters.

Placebos should not replace effective medical treatment, and secretly giving one to a patient can damage trust. Their real value may be what they teach us about medicine: treatment is influenced not only by a drug or procedure, but also by communication, confidence, previous experiences, and the relationship between a patient and clinician.

The placebo effect shows that the mind and body are not separate. Expectation cannot cure every disease, but it can influence how the body experiences illness and recovery.
